Prior to your appointment, please fill out this personalized online intake form to indicate any medical history, injuries or physical conditions. We’ll review together so that I can customize the appropriate treatment(s) to address any pain or tension points within your body or beauty goals you are looking to achieve.

Cancellations
No Show/Late Policy
I understand that unanticipated events happen occasionally in everyone’s life. To be effective and fair to all clients, the following policies are honored:
No shows and cancellations
I respectfully ask that you provide me with a 24 hour notice of any schedule changes or cancellation requests. Please understand that when you cancel or miss your appointment without providing a 24 hour notice, I am often unable to fill that appointment time. This is an inconvenience and also means my other clients miss the chance to receive services they need. I also reserve the right to require a credit card number in order to book appointments so that appropriate fees may be charged if a late cancellation or no show does occur.
Less than 24 hour notice will result in a charge equal to 50% of the reserved service amount. “No Shows” will be charged 100% of the reserved service amount.
Late Arrivals
If you arrive late, please note that your session may be shortened in order to accommodate others whose appointments follow yours.
Reminders
To better serve you, confirmations are sent via text and email 72 hours in advance of your session. Additionally, reminders are sent 24 hours prior to your scheduled appointment time.
